Eleven youths vanish from Mexico City bar, kidnapping suspected

MEXICO CITY (AP) ? Eleven young people were kidnapped in broad daylight from a Mexico City bar, just 20 days after the grandson of civil rights leader Malcolm X was beaten to death at a nightclub in the capital, anguished relatives said Thursday.

The disappearances sparked a wave of outrage, with family members blocking a major avenue to demand that authorities investigate.

The city prosecutor's office said Thursday only five missing-person reports had been filed. But the mother of one of the missing youths says 11 people in all vanished from the after-hours club about 1 ? blocks from the U.S. embassy, on the other side of Reforma Avenue.

The mother, Josefina Garcia, questioned how a mass kidnapping could occur a half-block from Reforma, the the city's main boulevard, and a few blocks from police headquarters.

Malcolm Shabazz, grandson of Malcolm X, died May 9 in a dispute over a bar bill.

U.S. households still feel the recession

WASHINGTON (AP) ? The average U.S. household has a long way to go to recover the wealth it lost to the recession, a report by the Federal Reserve Bank of St. Louis concluded Thursday.

The typical household has regained less than half its wealth, the analysis says. A separate Federal Reserve report in March calculated that Americans as a whole had regained 91 percent of their losses.

Household wealth plunged $16 trillion from the third quarter of 2007 through the first quarter of 2009. By the final three months of 2012, American households as a group had regained $14.7 billion.

Yet once those figures are adjusted for inflation and population growth, the average household has recovered only 45 percent of its wealth, the St. Louis Fed concluded.

That suggests that consumer spending could remain modest as many Americans try to rebuild their wealth by saving more and paying off debts.

The analysis, by St. Louis Fed economists Ray Boshara and William Emmons, noted that the rebound in wealth hasn't been equally distributed. As a result, many households are even further behind than the average.

Nearly two-thirds of the increase in household wealth since 2009 is due to rising stock prices, the authors note. Stock indexes reached record highs this month. Those gains disproportionately benefit affluent households: About 80 percent of stocks are held by the wealthiest 10 percent of the population.

For middle- and lower-income households, home values represent the biggest chunk of total wealth. And home prices remain about 30 percent below their peak, even after jumping nearly 11 percent in the past year.

"It's like the economy is this airplane and not all the engines are firing," Emmons said.

Still, wealthier households account for a disproportionate share of consumer spending: About 20 percent of Americans account for about 40 percent of spending.

Consequently, the rise in stock prices should continue to support overall consumer spending, Emmons said.

Origins of key cells in the thymus

May 30, 2013 ? Medullary thymic epithelial cells (mTECs) allow the thymus to ensure that the body's T cells are able to distinguish between potentially harmful foreign antigens and those that are produced by the body itself. A Swiss-Japanese research team suggests that mTECs do not share a common progenitor with cortical-thymic TECs (cTECs) that produce T cells, but may actually evolve from them.

T-lymphocytes, or T cells, are a principal component of the body's adaptive immune system. Together, these cells express a large repertoire of antigen specific receptors that recognise foreign material derived, for example, from pathogens and tumour cells. The generation of these antigen receptors occurs during T cell development in the thymus. This constitutes, however, a random process that also includes the formation of antigen receptors which respond well to the body's own proteins, so-called self-antigens. To prevent T cells bearing a self-reactive antigen receptor to exit from the thymus to the rest of the body where they may cause autoimmunity, a mechanism is in place that involves mTECs. These specialised thymic epithelial cells express most of the body's self-antigens. T cells that recognise their specific antigen presented by mTECs will undergo a process of programmed cell death and are consequently deleted in the thymus.

Very little is presently known about how cTECs and mTECs develop, or how they relate to each other. A Swiss-Japanese research team now reports that mTECs derive from cells that already express ?5t, a proteasome subunit that is densely concentrated in cTECs and no other cell types, including mTECs themselves. This finding, which is published in the May 27-30, 2013 edition of PNAS, suggests that mTECs may evolve from cTECs. This finding has not only implications for how mTECs develop, but also how they may have evolved.

The research project was led in Switzerland by Prof. Georg Holl?nder, Professor of Paediatric Immunology at the University of Basel and Action Research Professor of Paediatrics at the University of Oxford. In Japan, the project was led by Prof. Yousuke Takahama of the Institute for Genome Research at the University of Tokushima, which initially discovered the ?5t proteasome subunit. Dr. Izumi Ohigashi of the Institute for Genome Research at the University of Tokushima and Dr. Saulius Zuklys at the University Children's Hospital of Basel serve as first authors.

Broad Potential

Professor Holl?nder believes that the benefits of a better understanding of the origins and functions of mTECs and cTECs extend well beyond basic research. The team's findings suggest that evolutionary pressures have caused the body to check the quality of T cells that it produces. The T cell antigen receptor repertoire in evolutionary older species have a receptor, and did not require the body to implement quality control -- but as the capacity developed to produce a seemingly infinite number of T cell antigen receptors the vital need to control their specificities has arisen. For this purpose the body may have "hijacked" existing cells, namely cTECs. Holl?nder also believes that the findings could inform attempts to reconstruct or develop in-vitro thymuses, which could in turn be used to help people who lack a normal thymus function because of inborn or acquired defects. "You can fix things if you know how they are formed in the first place," he claims.

The research was supported by Grants-In-Aid for Scientific Research from the Ministry of Education, Culture, Sports, Science, and Technology and the Japan Society for the Promotion of Science, and by a Strategic Japanese-Swiss Cooperative Program on Molecular Medical Research from the Japan Science and Technology Agency and the Federal Institute of Technology (ETH)-Z?rich.

Roku raises $60M from Hearst, News Corp, others

(AP) ? Roku Inc., the streaming video device maker, said Wednesday that it has raised $60 million in its sixth round of funding. Media giant Hearst Corp. and an unnamed institutional investor joined existing backers such as News Corp. and its subsidiary British Sky Broadcasting in the round.

Roku's service offers an alternative to broadcast, cable and satellite television and first supported Netflix viewing five years ago. The funding round brings the total raised to date to $140 million and comes as the battle for the living room heats up as traditional pay TV subscriptions stagnate in the U.S.

Apple CEO Tim Cook told the All Things D conference Tuesday that his company has a "grand vision" for remaking TV. Microsoft Corp. last week unveiled a new gaming console, the Xbox One, that lets users can flip through channels using voice commands.

Roku says the funding will help it develop its video-streaming software and services business.

It has sold more than 5 million devices in the U.S. since its founding in 2008.

That compares with about 13 million for Apple Inc.'s Apple TV, a box that also lets people watch video from the Internet on their TVs.

Assad says Syria received Russian missile shipment: Lebanese media

BEIRUT (Reuters) - Syria has received the first shipment of a sophisticated air defense system from Russia, President Bashar al-Assad was quoted as saying, sending a signal of military strength days before an EU arms embargo on the country lapses.

Russia had promised delivery of the S-300 missile system to the Syrian government despite Western objections, saying the move would help stabilize the regional balance at a time of insurgency in Syria waged by Western-backed rebels.

Moscow is a staunch ally of Assad and it has appeared to grow more defiant since the European Union let its arms embargo on Syria expire as of June 1, opening up the possibility of the West arming the Syrian rebels.

"Syria has received the first shipment of Russian anti-aircraft S-300 rockets," Lebanese newspaper al-Akhbar newspaper quoted Assad as saying in an interview due to be broadcast later on Thursday.

More of the missiles would arrive soon, he was quoted as saying.

A source close to Russia's Defense Ministry said there had been a "bank transfer" in connection with the S-300 transaction but that Russian banks were becoming increasingly nervous about dealing with Assad.

"There were some problems with payments because big Russian banks were scared of dealing with Assad, but there was a bank transfer," the source said. "There are also not big banks and banks that are not based in Moscow. Beyond the down payment there was almost certainly a second payment, maybe a third."

Last year, a source close to Russia's weapons export monopoly Rosoboronexport said Syria had paid 20 percent of the contract price, but that the S-300 deal was frozen because of the country's civil war.

The United States, France and Israel have all called on Russia to stop the missile delivery.

More than 80,000 people have been killed in Syria since peaceful protests against four decades of Assad family rule morphed into a civil war, pitting the president's forces and his ally, Hezbollah, against Syrian rebels and a flow of Sunni Islamist militants who have come to help them from abroad.

Moscow says the expiry of the EU embargo complicates U.S. and Russian-led efforts to set up a peace conference between the Syrian government and its opponents, who demand an immediate end to Assad family rule.

GENEVA 2 CONFERENCE

The interview with Assad was to be released on Thursday on Al Manar, a television station linked to Assad's ally, the Shi'ite Muslim militant group Hezbollah.

Assad said he planned to go to the "Geneva 2" conference, al-Akhbar reported, though he was unconvinced of a fruitful outcome and pledged to continue fighting the uprising.

Officials in Israel, the United States' main ally in the region, say the S-300 could reach deep into the Jewish state and threaten flights over its main commercial airport near Tel Aviv.

Al-Akhbar said Assad also stressed ties between his forces and Hezbollah militants now openly fighting on the Syrian side of the Lebanese-Syrian frontier. "Syria and Hezbollah are part of the same axis," al-Akhbar quoted him as telling al-Manar.

"The Syrian army is the one fighting and leading the battles against the armed group, and this fight will continue until all those who are called terrorists are eliminated."

Syria straddles the fault lines of several regional and sectarian conflicts, and the violence in the country is increasingly seeping across its borders.

Israel, wary of any Syrian weapons being sent to Hezbollah in Lebanon, has already carried out three air strikes against Damascus to stop suspected transfers.

A recent series of strikes in Damascus, which shook the entire capital, stoked an angry reaction from Syria. State media outlets said Syria would respond to any further attacks and would also allow militant groups to attack Israel from a shared border on the Golan Heights.

Israel captured much of Syria's Golan Heights in a 1967 war and occupies the territory today, but Assad and his father, the deceased president Hafez al-Assad, had kept the border quiet for decades.

"The Syrian government will not stand in the way of any Syrian groups that want to wage a war of resistance to liberate the Golan," Assad was quoted as saying in the interview.

Hezbollah has said it is willing to support groups that chose to launch operations from the Golan.

Israel has become particularly wary of the S-300 shipments although Defense Minister Moshe Yaalon said on Tuesday that the shipments were not yet on their way.

Earlier this month, Israel was reported to have told Washington that Syria had begun payments for a $900 million purchase of S-300s, with an initial deliver due within three months.

Surge in hate crimes divides Greek coalition

ATHENS, Greece (AP) ? Greece's coalition government was in disarray Thursday over efforts to crackdown on growing racist violence, as majority conservatives and their center-left partners clashed over the best way to tackle anti-immigrant violence.

The embarrassing rift in conservative Prime Minister Antonis Samaras' government occurred amid pressure from the European Union and human rights groups to toughen anti-racism laws.

Greece, in its sixth year of recession ? and with the highest number of immigrants entering the European Union illegally ? has seen a sharp rise in violence against non-European immigrants, as well as a surge in support for the xenophobic and extreme right Golden Dawn Party.

Samaras' center-right New Democracy party refused to endorse draft legislation overhauling laws on racial equality, and instead submitted more limited amendments to laws passed in 1979.

But his coalition partners, the Socialist Pasok party and Democratic Left, submitted the more extensive proposals as a private members bill.

"What matters is that we unite political parties and the people against the Nazis, and not to let the danger of neo-Nazism divide us," Samaras said in a written statement.

Formed after general elections last June, the ruling coalition cannot pass legislation without some form of cross-party support.

The European Union, as well as international Jewish groups, have urged Greece to tackle racism by criminalizing incitement to commit racial violence among other measures.

The New York-based organization Human Rights Watch on Thursday also urged the swift adoption of measures against racial violence.

"With people being attacked on the streets, Greece urgently needs to beef up its criminal justice response to hate crimes," Judith Sunderland of Human Rights Watch said.

Opponents of the racism bill argue it could violate constitutional rights to free speech and free assembly. Meanwhile leftwing parties claim Samaras' conservatives fear antagonizing the far right as the country recovers from its crippling financial crisis.

Late Wednesday, several thousand Golden Dawn supporters attended a rally in central Athens. Holding Greek flags and fire torches, the crowd chanted: "Foreigners out of Greece."

Campaigning aggressively against immigration and Greece's bailout agreement, Golden Dawn has reaped a surge in support in recent years. The party elected 18 members to the 300-member parliament in last year's general election with nearly 7 percent of the vote.

An opinion poll for private Mega television published this week suggested support for the extreme right party has risen to 10 percent.

The GPO survey of 1,200 adults, conducted May 24-27, found two-thirds regarded Golden Dawn as a threat to democracy, and half supported the anti-racism law. Fewer than 40 percent of those who voted for Samaras' center-right New Democracy in the last election back the proposed legislation.

"There is no doubt that this law is targeting Golden Dawn," Golden Dawn leader Nikolaos Michaloliakos told his party's lawmakers ahead of Wednesday's rally.

"Let them bring the law to parliament and we will see, finally, who is with Greece and who is on the side of the illegal immigrants."

Golden Dawn denies any involvement in attacks against immigrants, though party supporters have been arrested as suspects in several recent incidents.

Looking for a Summer activity for your child? The South Bend Parks department is offering Summer sports programs in baseball, soccer, cheer and basketball!

Junior Baseball

This program provides an opportunity for parents, children and staff to interact and learn the basic skills of the game in a fun, non-competitive atmosphere. We offer morning games at Boehm Park and evenings at Belleville. Games last for one hour and are played at different times according to your schedule. Parents are encouraged to be involved on the field, help teach the basics while enjoying the program with their child. Junior Baseball consists of two different levels of play, based age categories. Join us for the Summer Cheer session. This is a 6 week cheerleading and dance camp that will teach basic cheer and pom routines to participants that will be show cased with a performance at the end of the program. Change, conflict cue memories of life's milestones

May 29, 2013 ? What will your kids remember about the life stories you tell them? New University of Alberta research shows that they're likely to be able to recall transitional moments you share with them, be it promotions or pets. The research offers strong evidence that societal values significantly affect how people think about and recall events in their lives -- and how we potentially carry old values and beliefs forward in a new country.

Psychology researchers Connie Svob and Norman Brown conducted interviews with two groups of participants, split evenly between people born in Canada and people whose parents emigrated from a country in upheaval. Each group was asked to identify the 10 most important events in their lives, when they occurred and whether the event had a psychological impact on them.

The results paint similar pictures of what people considered important, but showed striking differences in terms of the milestone events that often served as a backdrop.

"We were mainly interested in the historical context and how that gets transmitted," said Svob. "When a parent has lived through a historical event, how does that get passed on to the next generation -- and to what extent does it get passed on?"

Marking transitions: Dogs and divorce

Education, birth, death and marriage were among the top five major events mentioned by both groups, and most other major types of life events were separated by only a couple of percentage points.

What separated the groups were distinct elements or life markers that only appeared within one group. Seven per cent of what was labelled the conflict group recounted historical events their parents lived through, or their military service. Among the non-conflict group, six per cent cited attendance at a major sporting event, or the acquisition or loss of a family pet.

"We seem to be positively predisposed to detect and remember change," said Svob. "This cognitive capacity appears to extend to higher levels of cognition -- specifically, the ways in which we remember our lives and our histories."

Tying life to historical context: The Cup and coups

The researchers also asked participants to tie the events in their parents' lives to historical events. Svob notes that 25 per cent of the events mentioned by the conflict group were related to wars or other strife. For the Canadian contingent, the respondents anchored the memories to another type of conflict: sporting events.

"That's our history -- the Stanley Cup, the riots, the Olympics -- that's probably what we're going to remember," she said.

Thanks for the memories: Remembrance and public impact

Svob says Edmonton and the U of A were ideal places to undertake a research project like this, noting the city's rich and open ethnic diversity, and the institution's diverse international student base.

She says determining what people retain from their cultural history has benefits in terms of helping them retain their identity. But she notes that it also identifies lingering cultural issues related to ethnic out-groups -- issues that become important to manage, especially in Canada's ever-expanding cultural landscape.

She hopes the results from the study can be used to develop ways of bridging these narratives to the Canadian context, ensuring that newcomers thrive in adopting the peace and harmony of their new home.

"We were able to collect data that explore potentially contentious issues," said Svob. "It implies that the conflict-knowledge -- and its related attitudes and beliefs -- are carried forward among first-generation Canadians.

"To minimize xenophobia in Canada, interventions in schools and within other transition-related immigration programs could happen at the level of parental narratives concerning their war experiences."

Arrested Development, Season 4

In?Slate?s?Arrested Development?TV Club, two fans will IM about each episode of Season 4 once they finish watching it. Today,?Brow Beat?editor David Haglund and editorial assistant Emma Roller recap Episode 1, "Flight of the Phoenix."

David Haglund: So, you and I have both watched much more than just the first episode, yes?

Emma Roller:?Of course. I?m half-proud and half-embarrassed to say I finished the entire season in a day.

Haglund: Well done! I still have a few to go. Nonetheless, I thought maybe we could begin with first impressions of the first episode, before we get to our more considered, 24-hours-later reflections. At the beginning of Season 4, we learn that following the boat party fiasco that ended Season 3, Michael traded his shares in the family company to Lucille Austero, so that she could take it over; then he borrowed money to finish the Sudden Valley development, which sits entirely unoccupied, since there are no roads connecting it to the outside world. (Also: no Internet.) Finally, he moved into his son George Michael?s dorm room, a nearly broken man. Fun stuff! On first viewing, what did you think?

Roller: I don?t know what I expected. I liked the role reversal of Michael/George Michael, especially the switching of a line from the show?s pilot: ?What have we always said is the most important thing?? And unlike Michael?s smartphone calendar, the show isn?t stuck in 2003. I enjoyed the jab at the Bluths receiving stimulus money, only for George Sr. to spend it on land where he can build a border fence. The plot lagged as the episode went on, though?they spent the last third of the episode on voting Michael out of George Michael?s dorm room, which I thought was a bit overstretched. But it?s the poor carpenter who blames his shoddy tools, after all. What did you think?

Haglund: My reaction was similar. That vote-Michael-out-of-the-dorm-room storyline took forever! Even if it paid off in a couple of good gags at the airport. (?Well, of course P-Hound?s going to vote you out,? says the guy X-raying Michael?s luggage.) The freedom of the Netflix format mostly seems like a good thing, but? maybe if they?d had to cut this down to 22 minutes, they would have edited it a bit more ruthlessly. I was encouraged, though, by both the stimulus-funded-anti-immigration-wall scheme (these episodes are indeed taking place in Obama?s?or Boehner?s?America) and the bringing low of Michael Bluth. There were some amusing callbacks to the first three seasons, but they didn?t overwhelm the show. Kristen Wiig was delightful as a young Lucille, but Seth Rogen was inert as a young George, Sr. Poor casting there, I thought. And the episode highlighted the challenge of working around everyone?s schedules. These characters are funniest when they?re bouncing off one another, but we necessarily got much less of that.

Finally, is it just me, or are jokes about Michael Cera-Jesse Eisenberg confusion rather old hat? Granted, ?anti-social network? is pretty funny.

Roller: Huh. I like Kristen Wiig as much as the next tiny-handed person, but her Lucille portrayal didn?t do it for me?although that probably speaks more to the stubbornness of my love for Jessica Walter. And before I figured out that they were mocking The Social Network, I kept thinking, ?Man, why is George Michael such a jerk now?? And I definitely agree that the biggest problem?scheduling the old cast together?is an intractable one. That said, is it selfish that I don?t particularly care to meet any new characters?

Haglund:?I don?t know if it?s selfish or just overly nostalgic. Key to development of peripheral nervous system uncovered

May 28, 2013 ? Patients suffering from hereditary neuropathy may have hope for new treatment thanks to a Geisinger study that uncovered a key to the development of the peripheral nervous system.

In an article published today in the online medical journal Nature Communications, Geisinger researchers found that a protein present within immune system cells plays a larger role than previously thought in the development of the peripheral nervous system.

Nikolaos Tapinos, M.D., Ph.D., director of neurosurgery research and staff scientist at Geisinger's Sigfried and Janet Weis Center for Research, said the findings could have implications in how hereditary neuropathy is treated. Hereditary neuropathy affects the peripheral nervous system, causing subtle symptoms such as muscle weakness, wasting and numbness that worsen over time.

"When the peripheral nervous system develops in utero, certain proteins control how the cells travel throughout the body to the proper locations," Dr. Tapinos said. "Some of those proteins are already known, but this is the first time that the protein Lck has been identified as integral to this process."

Lck, or lymphocyte-specific protein tyrosine kinase, is a protein that is found inside specialized cells of the immune system. Dr. Tapinos' research found that Lck controls how cells called Schwann cells migrate across neurons throughout the peripheral nervous system.

Schwann cells function by creating the myelin sheath, the fatty covering that acts as an insulator around nerve fibers. In humans, the production of myelin begins in the 14th week of fetal development and continues through infancy and adolescence. When errors occur in the creation of myelin, hereditary neuropathy such as Charcot-Marie-Tooth disease (CMT), a motor and sensory neuropathy, can result.

"What we have found is that Lck is essentially the 'switch' that signals migration of the Schwann cells and production of the myelin sheath," Dr. Tapinos said. "This finding sets the stage for further research into the specific molecular mechanisms that occur in order for this process to break down, and eventually toward developing treatments to prevent it."

Nature Communications is an online-only, multidisciplinary journal dedicated to publishing high-quality research in all areas of the biological, physical and chemical sciences. Papers published by the journal represent important advances within each field.

Other authors on the article are Jennifer Ness Myers and Kristin Snyder, both of Geisinger's Sigfried and Janet Weis Center for Research.

?Out of the 11 individuals brought in, there were two 19 year old males, one 27 year old male, one 20 year old male, one 21 year old male, four 18 year old males and two 17 year old males,? police stated following the operation.

Local media reported at the time that the victim of the attack had fled into an electronics store in Male? in order to shelter from a group that proceeded to assault him using a ?sharp object?.

The store attack was among a series of stabbings reported across the capital this month ? ?prompting police to form a special task force designed to try and combat gang violence.

The now-active task force consists of members from the Forensics Directorate, Intelligence and Covert Policing Command, Information and Communication Directorate, Central Operations Command and Technical Police from the Divisional Operations Command of the Maldives Police Service.

Hameed advocated that a strong, more integrated, holistic approach is needed that applies a broader strategy to effectively reduce crime.

?Increasing police officers physical presence, as well as effective targeting of high profile offenders and hotspots needs to be addressed,? he said.

?Collaboration between criminal investigations, intelligence gathering, and front line policing are three areas that have never worked that effectively,? he added at the time.

iRadio: Apple could do streaming really well, but they don't have to do it at all

The rumors of an Apple iRadio music streaming service just won't go away. With WWDC just around the corner, many think that we may see something announced in San Francisco?in the very same room Google recently announced All Access. Apple could knock it out of the park if they entered this space, and I'm sure a lot of us would love to see just such an Apple branded service. But a couple of questions still present themselves. Does Apple really need to do music streaming, and what form could it take if they do?

Does Apple need to do a music streaming service?

The tl;dr version is; no. Apple doesn't need to do anything. Should, or even would, they is a better question. Apple reinvented the way we listen to music, making digital tracks work for record labels and consumers alike. For many, the iPod and iTunes were their first foray into Apple products. That's exactly where I began 9 years ago. The iPod brought with it the iTunes Store, and in the present day digital downloads have almost completely replaced the sale of physical media. And still, to this day, the largest source of digital downloads is the iTunes Store.

The iTunes Store is well established and does good business for Apple. It's also easy to understand. You give them money, they give you music. It comes with a sense of ownership. Apple currently offers iTunes in the Cloud, which lets you re-download previously purchased music, and iTunes Match, which lets you upload your local music to an iCloud locker and then re-download (but not really stream it) from any of your devices. In both cases, it's music you've already bought or acquired, Apple's just helping you access it. A streaming service, one with a subscription, is something else entirely.

Competitors who offer streaming also have music stores, like Google for example, but none are anywhere close to as established as iTunes. Google was late to the music store party, and has since thrown a streaming service on top of this. But, it's limited to the U.S. only?officially.

The leading streaming services have been designed to be that from the ground up. iTunes was built to sell music. Apple runs iTunes at just above break even, which granted at that scale is a lot of money, but they're not risk-adverse to obsoleting it. Their goal is to sell hardware. Music just encourages people to buy more hardware. Would subscription be even better at that?

What could Apple do with a music streaming service?

Apple being Apple, I can't believe they would move into the market simply offering the same as the competition. They've achieved their success by offering simpler, easier, and quite often, better.

The rumors point to some sort of iRadio service, and I think that would be a better place to start. Spotify and Google have radio features slapped on top of search systems for tracks, artists and albums. You find something you like, and you start a radio station based on that. Apple could do it better.

Over the years, customers have bought a ton of music from the iTunes Store, all of which Apple has the records of. They already use it for Genius Recommendations and Mixes. So, in theory, they could offer up a similar recommendation service that captures what you like to listen to. Twitter #music is a good example, and Apple could sit perfectly in the middle between Twitter and Spotify. Use all the iTunes information they already have, send you a radio stream of music they think you'll like, with an instant link to the iTunes Store to purchase the track, see more by the artist and so on.

Integration is something key to consider too, since a lot of this music would be listened to on mobile devices. If it needs to be a separate app, then it isn't done right. Apple already has a music app on iOS devices, so for something that they want their customers to adopt, it would need to be integrated into what they already have. And that could work, I'd be quite happy to open up the Music app on my iPhone and have the choice to listen to something new alongside my existing collection.

At the moment I live in Spotify and Twitter #music, but that's two additional apps on top of the stock music player. If Apple could bring all that under one roof, similar to how Google is trying to play it with their mobile apps and All Access, it could be a hit.

Patient communication has room to grow: studies

NEW YORK (Reuters Health) - There's room - and need - for improvement in the discussions between doctor and patient that go into medical decision-making, according to research out on Monday.

In four studies and a commentary published in JAMA Internal Medicine, the authors look at various aspects of doctors' dialogue with patients about prognoses, options and treatment preferences and find little consistency.

And though not all patients want the responsibility of making treatment decisions, medical organizations have long promoted the idea of patient-centered care through shared decision-making, and the 2010 Affordable Care and Patient Protection Act that goes into effect next year incorporates the idea into law.

What's more, "When physicians reach out and communicate to patients that their views are welcome, patients really like that a lot," said Floyd Fowler, senior scientific advisor for the Informed Medical Decisions Foundation in Boston.

But research in the past decade showed that U.S. patients with common medical conditions were not being adequately informed about their treatment options, so Fowler and his colleagues wanted to see if anything had changed recently.

They surveyed 2,718 U.S. adults who were over 40 years old in 2011 and had seen a doctor for any of the five most commonly treated medical conditions - high blood pressure and cholesterol, prostate and breast cancer screenings and back and knee problems - during the previous two years.

They found that doctors tended to discuss the pros and cons of surgeries, but not of cancer screenings or the choice of medication to treat high blood pressure or cholesterol.

"Each decision has its own dynamic, and it's intriguing that the heart risk related discussions weren't very good," said Fowler, the study's lead author.

He added that doctors may view back and knee surgeries as more complicated treatments, which is why they were discussed in more detail.

In a separate study of 207 kidney patients from two dialysis centers in Boston, researchers found that doctors rarely discussed very sick patients' prognoses or their eligibility for kidney transplants.

Dialysis patients tend to have one- and five-year survival rates comparable to those of many cancer patients, Dr. Melissa Wachterman, a palliative care physician with the VA Boston Healthcare system, and her colleagues point out in their report.

"Talking about prognosis is tough, but there are a lot of reasons why there is a benefit for patients to have this information if they want it," Wachterman told Reuters Health.

In their study, Wachterman's team interviewed 60 of the sickest dialysis patients and their doctors about expectations for the patients' survival and to what degree they had been discussed.

The researchers found only two patients whose doctors may have discussed their prognosis with them, and that more than half of the doctors said they would refuse to discuss prognoses - even if patients asked.

"I think we do a disservice to patients if we don't give them a sense that time could be short, because they have a lot that they want to do with the time they have left," Wachterman said.

In addition, the researchers found that while patients were good judges of whether or not they'd live another year, they tended to overestimate their long-term survival without their doctors' guidance.

"I think - in the long term - having this kind of information can actually give people hope and the ability to plan," Wachterman said.

But not everyone may want to play an active role in their care, according to Dr. Mack Lipkin, a professor of medicine at the New York University School of Medicine and the Bellevue Hospital Center in New York City.

"There are many people who want to be active participants in their care but there are also many people who prefer not to be so active," said Lipkin, who wrote an editorial accompanying the new research.

That's supported by a study of hospitalized patients, led by Hyo Jung Tak of University of Chicago, which found the vast majority wanted information on their illness and treatment options but more than 70 percent wanted doctors to make their medical decisions.

Moreover, Tak's group found that patients who preferred to participate in decision-making were more expensive. They stayed, on average, an extra quarter day in the hospital and racked up $155 to $1,576 more in costs.

Still, a study led by Dr. Harlan Krumholz, of Yale University School of Medicine in New Haven, Connecticut, surveyed 7,000 heart attack patients and found more than two-thirds preferred to play an active role in decision-making. A quarter of the respondents preferred to be the sole decision-maker.

At the very least, Krumholz's report concludes, doctors "who aspire to provide patient-centered care" should ask patients about their decision-making preferences directly.

Lipkin also said it's best to ask patients their preferences.

"We think the first thing to do is ask the patient what they'd like to know, how they'd like to learn it and then tell them," he said.

Fowler added that patients who want to be a part of the decision process shouldn't be afraid to speak up, and he hopes they get used to sharing in the process as the Affordable Care Act emphasizes shared decision-making initiatives.

"We're hoping these types of changes that are in the works really happen? and that when we repeat the study three or four years from now we'll start to see a difference. That would be great," he said.
